Colorado Doctors Plan

A collaboration between UnitedHealthcare, CommonSpirit Health, AdventHealth, HealthONE and Banner Health, Colorado Doctors Plan (CDP) is a different kind of health plan – one that focuses on your relationship with your doctor.

CDP is available to employees in the Denver metro area, including these 11 counties:

• Arapahoe
• Boulder
• Broomfield

• Morgan
• Weld

• Quality care from a doctor who knows you

  With the plans, you’ll need to select a primary care provider (PCP) – although each covered member of your family can choose their own if they like. Your PCP is there to guide your care, help you avoid cost surprises and support you in reaching your best health. When you’re ready to start looking for a PCP, just follow the steps in the Provider Search.

• Broad network coverage

  Choosing from the CDP network of providers and facilities is a key to getting cost savings and quality care. Just note that if you go outside the network for non-emergent care, you’ll be responsible for all the costs yourself.

• Lower copays and out-of-pocket costs

  The plans do include copays – the fixed amount you pay each time you see a network provider – but you’ll have a $0 copay for PCP, urgent care, online medical and behavioral health visits.

• No referrals required to see a specialist

  While your PCP may be in the best position to recommend specialist care, you won’t need a referral for a specialist visit.

• Preventive care at no additional cost with a network provider

  Along with wellness coverage, you’ll get a range of tools and resources to help keep you active and informed about your health.

• Pharmacy benefits through OptumRx

  You can order up to a 3-month supply of the medications you take regularly and have them delivered right to your home.

• Health Savings Account (HSA) with HDHP plan

  Your HSA is basically a personal bank account that lets you save money, pre-tax, to help pay for your out-of-pocket medical expenses. It may offer more flexibility than a Flexible Spending Account (FSA), which is offered with the DHMO plan.

Choice Plus

With the UnitedHealthcare Choice Plus plans, you have the freedom to choose your care from within or outside the network – without having to choose a primary care provider (PCP) or forfeiting coverage when you go out of network.
