Where to find providers

Our SignatureValue Alliance and Harmony HMO plans chose these network medical groups based on their technology and consistency of quality care, designed to help keep you and your family healthier — which may help reduce your health care costs.

The Alliance and Harmony HMO plans require you to select a PCP for you and each family member covered under your plan. Your PCP is your health guide – someone who can help connect you to the care you need and help you avoid cost surprises.

SignatureValue Alliance network providers

The medical groups in the Alliance HMO network have beenselected for their technology and consistent level of qualitycare. Choose from 35,281 physicians and 258 hospitals.1

SignatureValue Harmony network providers

With the SignatureValue Harmony network, you’ll have more choices in more neighborhoods. Choose from 17,000 providers and 175 primary hospitals1 in Los Angeles, Orange, San Bernardino, San Diego, Santa Clara, Santa Cruz and Riverside Counties.


How to use the Provider Search tool

Finding doctors, medical groups and facilities in your plan network is important, because staying in network gives you the highest level of coverage. Luckily, with the UnitedHealthcare Provider Search tool, it’s easier than ever.

  1. Go to the Provider Search page and select your SignatureValue plan to find doctor or hospital.
  2. On the next page:
    • Find urgent care facilities by selecting “View CA Medical Group and Urgent Care Information.”
    • Find doctors and hospitals by selecting “Continue.”
  3. If you know the doctor’s name or specialty, or the name of the facility, clinic or medical group, use the search bar.
  4. If you aren’t searching for a specific provider or place, select “People”, then “Primary Care.”
  5. Select “All Primary Care Providers” to see a list of doctors in your ZIP code.*
  6. Select the doctor’s name to find additional information – such as their address, phone number, hospital affiliation, reviews and whether they’re accepting new patients.
  7. Make note of the doctor’s Provider ID number, listed under the Locations tab. You’ll need it after you enroll in your plan.
